FIELD APPEARANCE & MEDIA RELEASE

I voluntarily authorize EIDA LLC ("EIDA") to photograph, film and/or audio-record me and to use my name (if supplied), image, likeness, voice, statements and performance (the "Material") in EIDA productions and related editorial, documentary, educational, artistic, promotional and publicity uses, in any media now known or later developed, worldwide, for the duration of applicable rights.

I understand that EIDA may edit, excerpt, combine, caption, translate, adapt and reproduce the Material, provided it is not used in a materially misleading or defamatory manner. I waive any right to inspect or approve the finished production or the specific use of the Material, and I understand that I will not receive compensation unless separately agreed in writing.

Privacy / data notice. EIDA LLC is the data controller for the personal information collected on this release and for identifiable recordings made with it. EIDA uses this information to document permission, produce and distribute media, administer rights, and protect legal interests. Material may be stored or processed in the United States and may be distributed worldwide. Release records may be retained as long as reasonably necessary to document EIDA's rights. Where applicable, data-protection rights may be subject to lawful exemptions for journalism, artistic expression or related publication. Questions or requests: hello@eidallc.com.
